That’s why Medicare patients in the greater DeWitt County area now have two additional programs to help them stay healthy. Chronic Care Management All five of our clinics are offering the first program, which is called Chronic Care Management. It’s for people who have at least two chronic health conditions. The program can help people with problems like arthritis, asthma, diabetes, heart disease or high blood pressure maintain their health between doctor’s visits. “I think it’s recognition of not what a patient does in the office, but rather what they do outside the office that helps keep them healthy,” says Betsy Harris, Vice President of Revenue Solutions for Revenue Maximization Group, which directs the program for the Cuero clinics. According to Harris, a personal Care Coordinator will connect with the Medicare patient over the phone to create a care plan, which includes goals specific to the patient’s conditions, as well as potential barriers to those goals. “That could include simple things like taking your medicines and whether you’re able to get to a pharmacy to pick up your medications,” Harris says. “Your Care Coordinator could help you find a way to get to the pharmacy if the need arises.” Free Annual Wellness Visit The second program for Medicare patients is a free Annual Wellness Visit. It’s a yearly checkup in which your Care Coordinator assesses your risk factors for disease, develops a personalized prevention plan and sets up a screening schedule for the year. Parkside Family Clinic and Cuero Medical Clinic began offering Annual Wellness Visits in January 2018. They’ll begin at Yorktown, Goliad and Kenedy clinics in March.
